SCENE

Four hours, face down

01
SCENE

07:40, the second case of the day. BMI 32, prone, thorax elevated 8 cm to keep ventilation patent. Induction is done. The surgeon is at the scrub sink. The remaining question: for the next four hours, who tends to this person's face, thorax, and ASIS?

02
REASONING

One of the most common complications of prone surgery is pressure injury. Face, thorax, and ASIS are the three peak-pressure zones. Studies show that when local interface pressure exceeds 32 mmHg for prolonged periods, perfusion drops and stage-1 injury can appear within four hours. The answer is not "softer" — too soft and the thorax sinks, compromising ventilation. The answer is "redistribute" — shift load from bony prominences onto adjacent, more tolerant tissue.
